PRSNM 

 

DD Waiver Standards provide the general foundation for services; however, each person’s Individual Service Plan guides person-centered service provision.  In addition, PRS-NM adheres to a higher standard of operational practices set forth by CARF.

In the provision of Customized Community Supports, PRS-NM offers adults with developmental disabilities the opportunity to develop social, recreational and vocational skills in a group or individual setting. Individuals collaborate with staff to develop a customized plan to address their own personal goals. Opportunities in the program include: Money Management and counting skills, Fitness groups using our Wii Fit for exercise classes, walking groups and Zumba Dance. We offer Music appreciation, Dance and karaoke for relaxation and creative forms of expression. Art activities including painting, drawing, photography, jewelry making and other crafts are also available. We also provide training in various life skills: Laundry, Cooking Classes, Kitchen Management, etc. Individuals also participate in Community Integration through Day Trips. 

PRS-NM provides non-medical transport for individuals to attend events, travel out of town, to visit their families who live out of town.  We have several individuals who have families that  live out of town.  Non-Medical transportation has been included in their budget so they are able to visit at least once a month.

Opportunities in the program include: Computer Skills Training, Job Etiquette, Money Management and counting skills, Fitness groups using our Wii Fit for exercise classes and Zumba Dance. We offer Music appreciation, Dance, Drumming classes and karaoke for relaxation and creative forms of expression. Art activities including painting, drawing, photography, jewelry making, sewing, and other crafts are also available.  

PRS-NM recognizes that individuals and families often need occasional breaks.  These breaks, in the form of out-of-home respite care, allow individuals receiving Respite care to have an enjoyable time meet new people and participate in a variety of fun activities.  Meanwhile, the family is provided relief from their ongoing caregiver responsibilities. PRS-NM provides families with an opportunity to have their family members stay in a safe, enjoyable environment through planned respite.  At the same time, the caregiver is able to take a few days off from care giving and perhaps go on a short vacation, attend a special event, spend time with other family members, finally finish a project, or simply relax.

PRS-NM provides services that enable individuals to live in their own homes or apartments with intermittent support this allows the individual to live more independently in the community. PRS-NM programs provide individualized support for adults on the DD Waiver. For Independent Living they include medical monitoring, including nursing, physical, BSC, occupational and speech therapies, with daily living skills training and assistance with participating in community activities. All these services are provided in the least restrictive manner to allow the individual the maximum amount of independence.

PRS-NM provides services that enable individuals to remain in their community setting with the option to interact with peers at the center and assists those people who transition from state-operated facilities into community settings. At PRS-NM we are flexible with group ratios providing small grousp  for those that need more assistance. PRS-NM is designed to provide supports for persons to gain and maintain optimum levels of self-determination and community integration while allowing flexibility in the provision of those supports.  PRS-NM encourages individuals to make community connection and meaningful relationships.

PRS-NM provides services that enable individuals to live in a community residential setting with intermittent support this allows the individual to live more independently.  PRS-NM programs provide individualized support for adults on the DD Waiver.  These supports include medical monitoring, including nursing, physical, BSC, occupational and speech therapies as directed by their needs, preferences and as outlined within their ISPs. PRS-NM provides assistance/instruction to the individual with activities of daily living including grooming, bathing, dressing, oral care, eating transferring, exercise, mobility and toileting. Assistance/instruction is also provided to help the individuals with the acquisition, restoration, and/or retention of independent living skills such as shopping, banking, money management, and the use of public transportation. PRS-NM assists with access to the community and assistance with acquiring or maintaining social interaction skills, community involvement and transportation.  These services are available up to 365 days per ISP year.  

PRS-NM ensures that each person in any of our Adult Nursing programs is provided professional and quality health services. Thorough health assessment, close monitoring and responsive follow-up, as well as on-call nursing. The agency nurse must work closely as part of a team approach with the physician, the interdisciplinary team members, direct care staff, case manager, services providers, family members, guardians and the individual to identify, coordinate and monitor the necessary health services. The determination of the degree of healthy supports needed by the person shall be solely decided upon a formal assessment process of the person served, as applicable. 

PRS-NM provides services that enable individuals to remain in their community setting with the option to interact with peers. At PRS-NM we also provide an option for individuals who need more support with daily activities. With CCS Individual we are able to provide these individuals with a one to one staffing ratio giving them the daily assistance they need. PRS-NM is designed to provide supports for persons to gain and maintain optimum levels of self-determination and community integration while allowing flexibility in the provision of those supports.  PRS-NM encourages individuals to make community connection and meaningful relationships.

 

PRS-NM provides services that enable individuals to live in a community residential setting with appropriate level of services and supports 24 hours per day, seven days per week. This allows the individual to live with the most independence within the community to prevent institutionalization.  PRS-NM programs provide individualized support for adults on the DD Waiver.  These supports include medical monitoring and nursing as directed by their needs, preferences and as outlined within their ISPs.  The supports are intended to increase and support independence and provide skills necessary to prepare the individuals to move out on their own.  PRS-NM provides assistance/instruction to the individual with activities of daily living including grooming, bathing, dressing, oral care, eating transferring, exercise, mobility and toileting. Assistance/Instruction is also provided to help the individuals with the acquisition, restoration, and/or retention of independent living skills such as shopping, banking, money management, and the use of public transportation. PRS-NM assists with access to the community and assistance with acquiring or maintaining social interaction skills, community involvement and transportation.  All services are provided in an integrated setting.  PRS-NM support staff monitors and provide assistance with adaptive equipment and assistive technology devices.  PRS-NM assists with access to the community and assistance with acquiring or maintaining social interaction skills, community involvement and transportation.  These services are available up to 365 days per ISP year.  Individuals may have roommates up to four in the home and in small groups while out in the community.

Adult Habilitation

Many individuals with developmental disabilities require specialized support to achieve community inclusion. PRS-NM programs provide individualized support for adults including those with complex medical needs. These programs combine medical monitoring and nursing with daily living skills training and active community involvement. Adult Day Habilitation programs are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).
